Fontana Unified School District Pre-Enrollment
PLEASE READ ALL BEFORE CONTINUING.
This is for students enrolling into kindergarten or grades 1 – 12 only.
Preschool, Prekindergarten and Transitional Kindergarten Enrollments need to be processed in-person at the Enrollment Center.
- Did you remember to electronically download and save the mandatory enrollment documents?
- Parent / Guardian’s Picture ID
- Proof of Residency
- Proof of Child’s Birth Date
- Child’s Immunization Record
- Child’s Physical Form OR Proof of an upcoming Physical Appointment (1st Grade & students coming from out of the country only)
- Did you remember to electronically download and save any additional documents that apply to your child?
- Previous School Transcripts or Records (High School Only)
- Special Education Document (IEP) – student is receiving Special Education Services
- Approved Intra-District OR Inter-District Transfer Letter
- Foster Placement
- Legal Documents
- Did you electronically complete, download and save the Student Enrollment Forms?

If you answered “NO”, to any of the three questions, please download and save what you need to before continuing.

This does not automatically enroll your child into school until an enrollment clerk verifies all documentation and contacts you via email with your next steps.
